How Do I Apply for a Prescriptive Incentive?


  1. Download the appropriate form from the list below.
  2. Read the form carefully to be sure your project meets the requirements to qualify for an incentive.
  3. Complete the project.
  4. Mail in the completed application form along with the invoice and any other information specified in the requirements to the address listed on the application form.
  5. Wait for your check; it should arrive within 6-8 weeks.
